Welcome to the Center for Women's Health! We would like to make this office visit as informative and comfortable as possible. If you have any questions or concerns you would like addressed, please write them down and bring the questions with you to your scheduled appointment. Should you so desire, spouses or family members are encouraged to accompany you on your visit. However, due to limited seating, we request that you bring no more than 2 visitors with you to your appointment. Children are welcome at the Center for Women’s Health, however, for their safety, please also bring another adult to supervise them during your examination.
It is necessary that you complete the accompanying paperwork and bring it with you to your scheduled appointment. Completing this paperwork prior to your arrival at our office allows our schedule to run on time, and decreases wait time for all of our patients. Faxing in your paperwork along with a copy of your insurance card prior to your appointment will help if this option is available to you. Sometimes we spend 30 minutes to an hour on the phone with insurance companies verifying elegibility. Please bring in your paperwork when you come. Due to the length of time required to complete paperwork, including medical history, your appointment may be rescheduled if the paperwork is not completed prior to your scheduled appointment.
If you have previous pertinent medical records, and your referring physician is not sending us a copy of your records, we request that you drop any records off to our office before your scheduled appointment, so that the physician has and opportunity to review them. We also request that you bring all prescription bottles with you to your scheduled appointment.
If for any reason you are unable to keep your appointment, please call our office to reschedule at least 24 hours in advance, so that someone else may benefit from the appointment slot.

ATTENTION YEARLY PATIENTS:
If you are being seen for a routine annual examination the following services may be performed:
Breast and Pelvic Examination
Pap Smear Collection
Dipstick Urinalysis
Hemoglobin
Hemmocult
Please check with your insurance carrier regarding your coverage for yearly preventative services and lab work.
PROBLEMS IN ADDITION TO YEARLY:
We will be happy to address any problems you are having at the time of your yearly visit. Please be aware that some insurance companies will not cover the problems when addressed at the time of the yearly exam. Other insurance companies require that you pay two copayments. Additional lab work may also be performed in order to address any problems.
